Rugby School Holiday Programme declaration
- I the parent/caregiver hereby give consent for my child (whose name appears on this Registration Form) to attend The Next Breed Rugby Holiday Programme.
- I understand that it is my responsibility to fully and adequately enquire of, and to disclose to The Next Breed Rugby HolidaysProgramme staff, any circumstancesthat could affect my child's safety oranyactivities I do not wish my child to participate in.
- I understand that some activities involve a risk of personal injury and/or property damage, and although The Next Breed staff will take all reasonable care, they will not be responsible for any accident/loss or injury suffered bymy child during the course of the holidays programme.
- I hereby authorise The Next Breed staff to organise medical or hospital treatment as they seenecessary, at my expense.
- I agree that if my child is not complying with The Next Breed rules or are behaving inappropriately, I will be contacted for thecollection of that child.
- I understand that all fees must be paid at the beginning of the day/week and I will receive no refund if my child iswithdrawn or unable to participate in the programme. All registration fees are non-refundable.
- I will complete the sign in/out sheet upon arrival and collection of my child.
- I will advise The Next Breed Staff in writing if my child is to be collected by any other person other thanthose listed on the registration form.
- Late penalty fees: The programme is not authorised to care for your child outside of programme opening hours.Late pickups past our extended hours will incur an additional penalty fee of $1 per minute after 4 pm.
- I understand that The Next Breed Rugby Holiday Programme retains the right to change or substitute activities when required.
