Programme Policy and Parents Permission Information:
- I acknowledge and bear the responsibility and risk to my child or children participating in the activities. I have explained to my child or children the need to follow instructions, remain in areas designated by staff and refrain from behavior that could cause injury. I acknowledge that any willful damage to equipment or property that has been caused by my child or children will incur an additional charge.
- I acknowledge that staff will to their utmost best, ensure safety at all times, yet recognize that accidents and injuries can still happen. I give permission for the qualified first aid staff to give any necessary medical treatment in the event of an injury. Any further medical attention will be at my own expense.
- I understand that teaching on Christian values is an integral part of the holiday programme and that in enrolling my child/ren I am agreeing to their participation in the associated activities.
- In the event of withdrawing my child/ren out of the programme I will be entitled to a refund of what I have paid less 15% administrative/booking fee.24 hours notice must be given in any event of withdrawing your child/ren in order for the refund policy to apply.
- If my child or children is unable to attend the programme on the day, I am responsible for contacting the programme coordinator and notifying absence.
- Our structured programme runs from 8:30 am 3:00 pm each day. Staff are not responsible for your child or children anytime outside of 8:30 am 3.15 pm. In the case that your child or children was still in our care at 3.15 pm and we had not any phone call explaining reasons, emergency contacts would be contacted, and you may remain in our care.
- All lost property will be displayed on the programme table at the end of each day. Any lost property that has not been claimed 1 week after the programme has finished will be placed in local clothing bins.
- Avonhead Community Trust programmes take no responsibility for loss or damage to any child or children's personal possessions and equipment (cd players, cell phones, PSPs, tablets etc). We recommend these items are left at home safe and sound. All cellphones, tablets, PSPs and other electronic devices will be held each day by the Programme Coordinator until the close of the programme day.
- I acknowledge that all information supplied on this application form will be kept on file, in accordance with the Privacy Act 1993. Information on your child/ren may be requested to be viewed at anytime.
- Information may be accessed by MSD (Ministry of Social Development), OSCAR (Out of School Care and Recreation), and CYF (Child Youth and Family) Auditors as part of the approvals process.
- I give permission for my child or children to walk to nearby Ferrier Park for activities, under the supervision of the leaders. (We have a pedestrian crossing that we use to get there safely).
- I understand that on any off site excursion, my child or children will be traveling either by bus or van to get to the location. In signing this, I am giving permission for my child or children to be transported in accordance to our policies and procedures.
- I realize that I am required to sign my child or children in and out of the programme daily and will notify staff if someone else is to pick up my child or children.
- At any time parents/caregivers are able to view the holiday programme policies and procedures
- We welcome your feedback via our feedback forms on the registration desk. If you have a complaint, please talk to the programme coordinator or email the Trust Chairperson: chair@actrust.org.nz
