Terms & Conditions for Kids Alive

I acknowledge and agree to the following conditions.

1: All children must be formally enrolled through Enrolmy

2: The Manager/Supervisor needs to be advised of any issues related to the collection of children, all children are to be collected no later than 5.30pm each day. Or if booked in till 3.00 pm at the Holiday Programme no later than 3.00pm. Parents will be given a 10 minute grace period if prevented from arriving on time. A penalty fee will be charged for failure to collect children, $1 per minute after 5.40pm. If it is more than 30 minutes then this will be $50.00 per child for every 30 minutes.

3:Fees must be kept up to date or paid in advance. Failure to do this can result in the child/ren being stood down until full payment is made.

4: Only persons appropriately identified as authorised people will be allowed to pick up a child unless the Programme Manager/Supervisor has been previously advised, preferably in person. If this is the case then the person maybe asked for photo identification before your child can be released.

5: Absences and Cancellations

After School Programme: The Manager must be notified if a child will not be attending on a booked day before 7am. Please text 027 4038126 (There is no absence fee and you will be charged the normal daily fee.) If you have booked your child in and then you need to cancel, then we require one weeks notice to enable a full refund. If you cancel after this but before the week of the programme commences (eg Sunday before 2.00pm) then 50% refund will be given. If no notification of absence is given and staff have to look for your child/children or call you then you will be charged the casual fee for that day.

Holiday Programme: Please advise Kids Alive by texting 027 4038126 before 8am. If we are not informed and have to phone the parent/caregiver to ascertain if the child is coming, an extra charge of $10.00 for that day will be charged. If you have booked your child in and you need to cancel then we require one weeks notice prior to the Holiday Programme starting to enable a full refund. If you require to cancel after this time but prior to the programme commencing (Sunday before 2.00pm) then 50% will be given. Any cancellation after this time no refund will be made.

For the December or January Programmes where the programme runs for more than one week, 10 days is required for a full refund and 50% will be paid with one weeks notification. After this time there will be no refund.

6. When applying for WINZ subsidies it is the parents responsibility to pay the Programme fees until authorisation from WINZ has been received. This will then be refunded from the date that WINZ begins payment.

7: Any disagreement regarding fees is to be addressed to the Programme Manager.

8: The Programme Manager must be advised immediately of any change in details contained in the Enrolment form.

9: The Programme Manager will be advised of any situation that may be disturbing to the children.

10: It is ultimately my responsibility to ensure that my child/ren act in an appropriate manner while at Kids Alive. Behaviour which consistently affects the quality of care to other children may result in suspension or removal from the programme as set out in the Behaviour Management Policy.

11: The Programme Manager may arrange for urgent medical treatment at my expense.

12: I agree to pay for damage wilfully caused by my child.

13: I understand my child is responsible for his/her own property.

14: I give permission to go to prearranged outings.T his may include trips to the skate park, playground and other public places. These will be either walking or transported by bus/mini bus. I understand that at all times the staff/child ratio will be adhered to.

15: I understand that all child/children and parent information is stored on Enrolmy and is secure under the Privacy Act 2020. Staff will have access to this information on the Enrolmy App or website. . Under the accreditation process, access to the child/children and parent information maybe viewed by Te Kāhui Kāhu during the review of the program.

16 Photographs of my children may be used for genuine resources and publicity purpose unless I have advised otherwise on my child's enrollment form.

17: I give permission for my child to watch the occasional video, DVD, rated G or PG only.

18: I agree to abide by the rules and procedures of Geraldine Family & Youth Trust Kids Alive (a copy of all current policies and procedures can be obtained from the Manager).

19: I understand that all staff are screened, trained and are gaining appropriate experience for the job they do and Holidays @ Kids Alive complies with Health and Safety Policies and the National Oscar Standards. However accidents do happen and I will not hold the Geraldine Family & Youth Trust, the operators of Holidays @ Kids Alive responsible for genuine mishaps.

20: I understand that the Kids Alive programme is managed by the Geraldine Family & Youth Trust which is completely separate from the Geraldine Primary School. Neither the staff nor the Board of Trustees of Geraldine Primary School are responsible for the Kids Alive service, nor liable for any act or omission arising through its activities.
