Welcome to Glenbervie ASC.
We aim to provide a safe and stimulating programme for your child/ren to enjoy during the time they are at Glenbervie ASC.
If you wish to enrol your child/ren at Glenbervie ASC, please read thoroughly the following conditions.
Our Policy and Procedure booklet is available at Glenbervie ASC for you to view at anytime.
Parent Responsibilities:
1. All children including those attending on a casual basis must be formally enrolled before attendance.
2. The supervisor will be advised immediately of any change to the enrolment form.
3. All children are to be collected no later than 5.30pm each day. A penalty of $10.00 per10 minutes will
be charged for failure to collect your child by closing time. This is to cover staffs extra wages.
4. The supervisor is to be advised if someone other than the appointed person is collecting your child/ren.
No child/ren will be released to an unauthorised person.
5. Children must be signed in/out each day. I will advise Glenbervie ASC if my child is not attending prior to the
start of the session.
6. Fees are to be paid weekly/fortnightly unless arrangement is made with management. All fees must be paid in full at the end of term.
7. WINZ applications must be presented to WINZ and approved prior to commencement.
8. Any unpaid account that is sent to the debt collection agency will incur further fees, which is the
responsibility of the debtor.
9. Cancellations to bookings will be charged at 75% of the regular fee. If you fail to cancel and we have to contact you our full fee applies.
10. We require one weeks notice to amend bookings.
Concerns and Disputes;
1. A full copy of the programmes operating procedures and policies, including for voicing concerns or complaints is
available at the programme. Any concerns or queries should initially be directed to the supervisor.
2. Behaviour that consistently affects the quality of care available to other children may result in dismissal from
the service, after other possibilities have explored.
3. Any disagreement of fees or other concerns may be discussed with management.
Duty of Care/Parental/Guardian Liability
Under our Duty of Care policy all care will be taken by Glenbervie ASC to provide supervision of children attending Glenbervie ASC in accordance with programme policy and procedures. (A copy of our policies and procedures is available to parents/guardians).
I (the parent/guardian) acknowledge nevertheless that in signing this form, that neither the staff nor management of the programme will be liable for any loss or damage (by way of accident, injury, theft or otherwise) arising out of attendance at the programme.
The Glenbervie ASC service is completely separate from the Glenbervie Primary School, although it has been established to benefit the families attending Glenbervie School. Neither the staff, nor the Board of Trustees of Glenbervie School, are liable for any act arising through Glenbervie ASC activities.
In signing this agreement you are acknowledging that you have read and understood the terms and conditions of the agreement. You are also acknowledging that you have been given an opportunity to ask us any questions you may have before signing the agreement. If you have any questions regarding the programme, Complaints, Code of Behaviour, And/or the Prevention of Child Abuse Policies etc please refer to the Programmes Policies and Procedures, which are available upon request.
PRIVACY ACT 2020: Information is collected to ensure the safety and well being of children at the programme and will not be used for other purposes. All information about individuals is available to them for review at any time. As the monitoring agency for OSCAR Services, Ministry of Children or Oranga Tamariki - Ministry of Children, may require access to this enrolment information.
